1. The first line of the application of the insured person for the appointment of the funded part of the labor pension (hereinafter referred to as the application) shall indicate the name of the non-state pension fund to which the insured person applied, in full in the nominative case.

2. Paragraph 1 of the application shall contain information about the insured person who is assigned the funded part of the labor pension:

surname, name, patronymic (if any) of the insured person are indicated in full in the nominative case in accordance with the document proving his identity;

in the line "Insurance number of the individual personal account of the insured person" the insurance number of the individual personal account of the insured person is indicated in accordance with the insurance certificate of the obligatory pension insurance of the insured person;

in the line "Number of the pension account of the funded part of the labor pension" shall indicate the number of the pension account of the funded part of the labor pension of the insured person;

in the line "Belonging to citizenship:" the word "citizen" or "citizen" is written, depending on who this line is filled in for, and the state of which the person is a citizen (subject) is indicated. If a person has dual citizenship, information about belonging to citizenship is indicated separated by a comma. 3. in the line "Phone number" the telephone number of the representative of the insured person (the organization entrusted with the performance of the duties of a guardian or trustee) is indicated;

in the line "Name of the identity document of the representative" the identity document of the representative (passport, residence permit or other identity document established by the legislation of the Russian Federation) is indicated, then in the corresponding lines of the application the series, number, date of issue of the identity document are indicated , name of the authority that issued it;

the line "Name of the document confirming the authority of the representative" shall indicate the document on the basis of which the representative performs his functions on behalf of and in the interests of the insured person (certificate of guardianship or guardianship, power of attorney or other document confirming the authority of the representative to exercise guardianship or guardianship established by law Russian Federation), then in the relevant lines of the application, the series, number of the document (if these details are provided for in the form of a document confirming the authority of the representative), the date of issue and the name of the authority that issued it are indicated.

4. Clause 3.1 of the application is filled in if the insured person has pension savings accounted for in the pension account of the funded part of the labor pension, formed from additional insurance premiums for the funded part of the labor pension, employer contributions, contributions for co-financing the formation of pension savings, income from their investment, funds (part of the funds) of maternity (family) capital, aimed at the formation of the funded part of the labor pension, income from their investment.

Depending on the choice of the insured person, whether or not to take into account the indicated funds of pension savings as part of the funded part of the old-age labor pension, a mark is made in one of two boxes: "take into account", "do not take into account".

5. In paragraph 4 of the application, in the appropriate box, a note is made on the appointment or non-assignment of the old-age labor pension, the insurance part of the old-age labor pension (with the exception of the fixed basic amount of the insurance part of the old-age labor pension), and also indicates the date (day, month and year ), with which the insured person was assigned an old-age labor pension, or insurance part old-age labor pension (with the exception of the fixed basic amount of the insurance part of the old-age labor pension).

6. In paragraph 5 of the application, depending on whether an urgent pension payment was assigned or not, a mark is made in the corresponding box.

7. Clause 6 of the application contains information about which the insured person (his representative) has been warned;

in the column "Name of the document" the names of the documents attached to the application are indicated. If several documents with the same names are submitted, in addition to the name, details (information) clarifying the document are indicated: date of issue, number, for example, "certificate of seniority dated 09.11.1979 N 365";

in the line "I am familiar with the information specified in paragraph 6 of the application:

9. A receipt-notification of acceptance and registration of an application of an insured person for the appointment of a funded part of a labor pension (hereinafter referred to as a receipt-notification) is filled in by an employee of a non-state pension fund who accepted an application from an insured person (his representative), except for the line "Receipt-notification received (and )" containing the date, the signature of the insured person (his representative), the transcript of the signature, in two copies, one of which is handed over to the insured person (his representative) or sent to him by mail.

10. The name of the non-state pension fund to which the application has been submitted shall be indicated in the first line of the receipt-notification.

11. In paragraph 1 of the receipt-notification:

in the line "Statement of the insured person on the appointment of the funded part of the labor pension" the surname, name, patronymic (if any) of the insured person are indicated in full in the nominative case in accordance with the document proving his identity;

in the line "insurance number of the individual personal account of the insured person" the insurance number of the individual personal account of the insured person is indicated in accordance with the insurance certificate of compulsory pension insurance;

in the line "submitted in the interests of the insured person by his representative" the surname and initials of the representative of the insured person shall be indicated;

in the line "and documents submitted with the application" the documents accepted simultaneously with the application are indicated:

In the column "Name of the document" the names of the documents attached to the application are indicated. 12. Paragraph 2 of the notification receipt indicates the documents that must be submitted additionally for the appointment of the funded part of the labor pension:

in the line "Documents that must be submitted additionally no later than __________", the date is indicated no later than which additional documents must be submitted for the assignment of the funded part of the labor pension;

in the column "N p / p" the serial number of the record about the name of the document is indicated;

in the column "Name of the document" the names of the documents are indicated (certificate of seniority, etc.).

13. In paragraph 3 of the receipt-notification in the line "Receipt-notification received (a)":

in the column "Date" the insured person (his representative) indicates the date of receipt of the receipt-notification;

in the column "Signature of the insured person (his representative)" the insured person (his representative) puts a signature, which confirms:

Receipt of receipt-notification;

Obtaining an explanation of what documents must be submitted additionally for the appointment of the funded part of the labor pension and within what time frame;

14. Clause 4 of the receipt-notification contains information about which the insured person (his representative) is warned.

15. Item 5 of the receipt-notification is filled in by an employee of the non-state pension fund when the insured person (his representative) submits additional documents after receiving the application of the insured person for the appointment of the funded part of the labor pension in the manner prescribed for filling out item 1 of the receipt-notification. At the same time, points 1, 2 and 3 of the notification receipts are not filled out.

The line "Last document submitted (date)" is filled in if the additionally submitted document is the last document required for assigning the funded part of the labor pension.
